Form Usage & Submissions

This guide covers the "Usage" tab in the Form Designer and the organisation-wide submissions view.

Step 1: Form Submissions lists every submission for this form with the submitted values laid out by the form's current field labels, plus location, page (if embedded), source details and whether a lead was created. Click a row to open the submission detail, which includes the IP address, user agent, referrer URL and the consent snapshot recorded at submission time.

Step 2: Location Association shows the location set on the Setup tab, or a note that the form is not associated with a location and submissions will be linked to the organisation only.

Step 3: Pages Using This Form lists every Dash Page that embeds this form through a Form widget, with the page's status and a View button. A form embedded on a page inherits that page's location only when the form itself has no location (see Setting Up a Form → Location). To embed a form on a page, open the page in the Page Designer, Build Your Page tab, and add a Form widget.

Step 4: To review submissions across all forms, go back to Data → Forms and choose "View Submissions" from the actions (gear) menu in the top-right. The same filter bar (Client, City, Location, Form Type, date range) applies.

Changes to Source, Source Details, Lead Type and Location apply from the next submission onward; they never rewrite historic submissions or leads.
